It caused by excessive pronation of the foot. Lateral stress causing subluxation over a period of years establishes a new position for the patella, creating an uneven pressure on the lateral surface. The shape of patella changes as it adapts to these stress and new position. Abnormal pronation causes internal rotation of the tibia, this produce pain.
Women , in particular, suffer with this problem because of their anatomical disadvantage, i.e. wider hips.

Treatment: Orthotics can realign the foot and prevent the changes that occur at the knee.


      Leg Length Discrepancies

Symptoms: Lower back pain, bursitis on the knee, medial shin splints, Achilles tendon pain and ankle pain are some problems.
Compensation for leg length difference can take place in the feet. On the short side, the foot will supinate and maintain weight-bearing on the outside of the foot in an attempt to lengthen the limb. There will be excess wear on the inside of the shoe as the foot pronates and shortens that limb.A runner complaining of unilateral knee pain should immediately suggest a leg length discrepancy.

Treatment: Physiological leg length difference may require an orthosis as well as a heel lift for
poor foot mechanics.
